Right to Refuse Service

We reserve the right to refuse service to anyone.

Professional Fee Terms

The Professional Fee is for the Prescribing Provider's time and experience, not for any specific medication.

Emergency Suboxone Availability

Arrow Dark Image

A three day emergency prescription for Suboxone is available for eligible non-patients of Clendenin MD.

Chronic Pain Policy

We do not treat chronic pain. Seek a pain management physician for this.

Subutex Policy

We do not prescribe Subutex except for special circumstances.

Controlled Substance Limitations

We do not prescribe stand-alone Opioids, Benzodiazepines or Tranquilizers.All Scheduled Controlled Substances must come from and be managed by a Prescribing Provider with Clendenin MD.

Mental Health Conditions Not Treated

We don’t currently treat Schizophrenia, certain personality disorders, severe mental illnesses that require inpatient care, or patients looking for therapy only.

Ketamine and Substance Abuse History

We will not use Ketamine for patients who has a history of substance abuse.

Late & No-Show Policy

More than 10 minutes late for scheduled appointment will be considered a no show and there will be a $25 rescheduling charge.

Suboxone & Benzodiazepines Interaction Policy

Suboxone patients are not allowed to take Benzodiazepines concurrently.

Age Restriction for Care

Our telemedicine services are available only to individuals 18 years of age and older. We do not provide care to minors.
